Product development for the real world requires a serious
amount of testing.
Having a testing team with all the equipment, knowledge,
testing tools, etc can be expensive and time consumeing. Then you have to test
it in the real world where you find out the industry does not always "work" as
expected.
Some of the major vendors clearly violate the DICOM
standard.
